Where will my visit take place?
Your visit will be over the phone. Your Homeward Care Navigator will call you at the time of your visit.
What to expect during my visit?
Your Care Navigator will review your health history so they can discuss services that will best support you.
During your visit, you will be able to:
Talk about any concerns or challenges on your mind—health, transportation, food, or other needs.
Learn about Homeward's services, like help finding doctors, scheduling appointments, or setting up your patient portal.
Explore ways to make the most of benefits you already have, such as over-the-counter benefits, mail-order pharmacy, or fitness programs like SilverSneakers.
What happens after my visit?
Before the Care Navigator leaves, we will schedule your next Homeward appointment to keep you on track. We will send your appointment notes to your primary care provider.

No, Homeward works with your existing doctors and specialists to coordinate your care. If you don’t have a primary care doctor or are looking for a new doctor, Homeward can get you connected with one.
