They will follow-up after Dr. visits to make sure everything is going well, help you schedule appointments and connect you with local services. They will help you navigate your care, make health goals and establish plans to reach them.

Yes. Homeward works with your existing primary care doctors and specialists to help you meet your health goals. But care goes beyond a doctor’s visit, and we can also help with things like follow-up care, medication questions, insurance benefits, connecting with local services like meal delivery, organizing rides and household chores, and more.
