Homeward Virtual Provider Visit 101
A virtual visit is a convenient way to get care from Homeward Health without leaving your home. Video is used so that the Homeward provider can complete a thorough health assessment. You’ll need a smartphone, computer, iPad, or tablet with a camera.
To ensure you make the most of your visit, please review this information about how to prepare for the visit, getting connected online, and what to expect.
What to expect during your visit
You’ll meet on video with the Homeward provider so that they can complete a thorough health assessment and you can:
Get answers to your health questions or concerns
Discuss tips on how to manage a chronic condition
Review your medications and any changes
Discuss any necessary preventive screenings or upcoming appointments
Review local services and resources available to you including insurance benefits
How to prepare for your virtual visit
Step 1: A few days before your appointment do the following:
Ensure you’re on time to your appointment and check that the device you’ll be using is compatible and working properly. Just click on the link below and then follow the prompts:
Step 2: The day before your appointment:
Confirm the date and time of your visit.
Have your medications and vitamins ready to review with your Care Team, as well as your driver’s license and insurance card.
Write a list of any questions you would like to discuss.

Keep your phone nearby.
Make sure the device you’re using for the visit has a strong internet or data connection.
Step 1: Your Homeward provider will call you on the phone at your appointment time.
Your visit with a Homeward provider will take place online.
At the time of your visit, the Homeward Care Team will call you on the phone to get started. They will make sure you receive the link to begin the virtual visit portion. Once confirmed, you’ll hang up to start the video portion of the visit with the Homeward provider, using the link sent to you.
Step 2: Using your smartphone, ipad, table or computer device, click on the link provided to you.
The link will take you to a browser on your device. You should see a screen like this.
⚠️ If you don’t see this screen or get an error message, you may need to use a different browser.
Step 3: Fill in the check-in information.
Type your full name, select “Yes, I am the patient”, and check the box acknowledging that you agree to the Terms of Use and Informed Patient Consent.
Then, click “Next”.
Step 4: Allow the browser to use your microphone and camera.
If you see a pop-up like these images, select “Allow”. This lets your provider see and hear you during the virtual visit.
Step 5: Stay in the Waiting Room until your provider joins.
If you see this screen, then you’re all set! Stay on this screen and the provider will appear shortly to start your visit.
If the system detects any technical issues, it may give you additional prompts. If you cannot resolve those issues, call Homeward at (844) 578-4535.
What happens after your visit?
At the end of your visit, we’ll schedule your next Homeward appointment to keep you on track. We’ll send your visit notes to your primary care provider. You will also receive the After Visit Summary (AVS) within two weeks after your appointment. It will be mailed to your home address.
