Care Navigation is an important service, whether you are healthy and looking for preventative care or whether you’re managing complicated chronic conditions. Your personal Care Navigator can help you get access to a range of services - both for medical needs and non-medical, like transportation, food delivery, or accessing mental health support.

Homeward’s Care Navigators are a dedicated team of nurses, doctors, and local experts to serve as a resource for our Members. They act as an advocate for your overall health and work with you to make a personal plan for your own goals to lead a healthy and happy life.

Locating in-network doctors and mental health providers in your local area.

Scheduling appointments and screenings, with the option of consulting with Homeward's own doctors or nurses in coordination with your existing doctor.

Facilitating access to local resources such as rides, meals, caregiving support, and more.
