Senior Vice President of Value-Based Care Operations and Analytics

Sapna Swaroop, Ph.D., is Senior Vice President of Value-Based Care Operations and Analytics at Homeward. Prior to joining Homeward, Sapna served as Senior Vice President of Value Based Care at Livongo, as well as Teladoc, where she led the development of value-based care models in chronic condition management and virtual primary care.

Sapna began her career at the Minnesota Department of Health as the Evaluation & Research Manager where she launched the Statewide Health Improvement Program, a $47 million public health effort focused on chronic disease prevention and health equity. Sapna then joined Blue Cross and Blue Shield of Minnesota and held several roles to support Blue Cross’s efforts to improve clinical quality outcomes for Medicare, Medicaid, and Individual & Family Plan beneficiaries. She was part of the founding team in the Star Rating & Risk Adjustment Center of Excellence. 

Sapna completed her bachelor’s degree in Sociology at the University of Chicago and received her master’s degree and Ph.D. in Sociology & Demography at the University of Michigan. She received her post-doctoral training at the Population Research Center at the University of Chicago where she completed academic research that investigated how race, economics, and geography intersect to shape health in Chicago communities, paving her career and commitment to advance health equity.
